      目的 评估经牙槽嵴顶入路上颌窦底内提升术同期植入短种植体的临床效果。方法 根据纳入及排除标准筛选病例,共筛选出27位患者,分别在骨量不足的上颌后区通过牙槽嵴顶入路采用上颌窦底内提升、不植骨的方法植入33颗短种植体。观察测量术前,术后即刻,术后6个月,术后1年短种植体周围成骨情况以及上颌窦底牙槽嵴高度变化。对测量数据进行描述性、均数、配对t检验等统计学分析。结果 上颌窦底牙槽骨平均高度为术前(5.56±1.70)mm,术后即刻增加至(7.76±1.54)mm,术后6个月为(6.94±1.44)mm,术后一年为(6.83+1.53)mm。采用配对t检验对各时间点测量值两两比较,观察各个时期牙槽嵴高度的变化。术前与术后即刻相比牙槽嵴的高度增加了(2.19±1.34)mm;术后6个月与术后即刻相比牙槽嵴高度减少(0.82±1.26)mm;术后1年与术后6个月相比,牙槽嵴厚度减少了(0.11±1.17)mm;术后1年与术前相比,牙槽嵴高度增加了(1.31±1.74)mm。上颌窦底牙槽嵴的高度除了术后1年与术后6个月无显著变化之外,其余各时间点的配对t检验差异均有统计学意义(P<0.05)。结论 上颌窦底内提升术同期植入短种植体临床效果确切,手术创伤小,是针对上颌后牙区缺牙伴骨量不足的有效治疗方案。

      Objectives The aim of this study was to evaluate the clinical effects of maxillary sinus floor elevation with simultaneous implantation of short implants. Methods A total of 27 patients were selected according to the inclusion and exclusion criteria. 33 short implants were implanted in the posterior maxillary area with insufficient bone mass through the alveolar crest approach by maxillary sinus floor elevation without bone graft. The bone formation around implant and the height of maxillary sinus floor changes were measured at four point of time:before surgery, immediately after surgery, 6 months and a year after surgery. Means of the measured data and paired t test were analyzed and statistical significance was evaluated. Results The average of observation period was (17.42±4.84) months and no failure case occurred during the period. The measured result before operation immediately after surgery, 6 months after surgery and a year after surgery was (5.56±1.70) mm, (7.76±1.54) mm; (6.94±1.44) mm and(6.83+1.53) mm, respectively. Statistical results by paired t test showed that the height of alveolar ridge increased obviously by (2.19±1.34) mm immediately after implant surgery. The height of alveolar ridge decreased by (0.82±1.26) mm at 6 months after the operation compared with that immediately after the operation. The thickness of alveolar ridge decreased by (0.11±+1.17) mm at 1 year after surgery compared with that at 6 months after the operation. Compared with that before operation, the height of alveolar ridge increased significantly by (1.31±1.74) mm at a year after surgery. The height of the alveolar ridge of the maxillary sinus floor had significant changes in all groups except that for 1 year and 6 months after operation. Conclusion Simultaneous implantation of short implants in maxillary sinus floor elevation via crest approach is an effective treatment for maxillary posterior edentulous region with insufficient bone volume.
