目的 探讨慢性阻塞性肺疾病中牙槽骨骨组织破坏与牙周膜VEGF-A165a表达水平的关系。方法 以11例单纯慢性牙周炎(CP)和12例慢性阻塞性肺疾病合并牙周患者(COPD+CP)为研究对象,分离培养2组拔牙病例的牙周膜成纤维细胞,制备条件培养基诱导破骨细胞(Trap+MNCs),提取RNA、逆转录及realtime PCR,检测病变牙周膜细胞中VEGF-A165a的相对表达量以及对应的破骨细胞诱导效率,并采用Perason相关性分析法评估两者的相关性。结果 COPD+CP组比CP组表达更高的VEGF-A165a水平,并可以诱导更多的破骨细胞形成(P<0.05)。重度和极重度COPD患者的牙周膜成纤维细胞中VEGF-A165a的相对表达量和破骨细胞诱导效率皆高于轻度COPD者。两组的VEGF-A165a的相对表达量与破骨细胞诱导效率成正相关性(P<0.05)。结论 COPD合并CP患者的牙周膜成纤维细胞中VEGF-A165a表达量和破骨细胞诱导效率均高于CP患者,该结果为临床干预COPD合并慢性牙周炎的治疗研究提供了一个潜在靶点。 |

Objective The aim of this study was to investigate the role of VEGF-A165a in alveolar bone destruction in patients with chronic periodontitis (CP) and chronic obstructive pulmonary disease combined with periodontitis (COPD + CP). Methods Periodontal ligament fibroblasts were isolated and cultured from patients with COPD + CP and CP respectively. Conditioned medium was prepared to induce osteoclasts (Trap + MNCs). RNA extraction, reverse transcription and realtime PCR were performed to detect the relative expression level of VEGF-A 165a.The t-test and Perason correlation analysis were performed to evaluate the relationship between the expression level of VEGF-A165a and the efficiency of osteoclast induction in the two groups of diseases. Results COPD + CP group expressed higher levels of VEGF-A165a alternative splicing isoforms and induced more osteoclast formation (P<0.05). According to COPD pulmonary function classification, the expression levels of periodontal ligament fibroblasts, VEGF-A165a alternative splicing isoforms and osteoclast induction efficiency in patients with severe and very severe COPD were higher than those in the mild COPD group. The relative expression of VEGF-A165a was positively correlated with osteoclast induction efficiency in both groups. Conclusions The expression level of VEGF-A165a and the osteoclast induction efficiency were both higher in patients with chronic obstructive pulmonary disease than that in patients with chronic periodontitis, providing a potential target for clinical intervention in chronic obstructive pulmonary disease combined with periodontitis research. |
