| 目的 评价Er:YAG激光联合龈下刮治根面平整术（SRP）治疗慢性牙周炎的临床疗效，并探讨牙周相关指标等因素的影响。方法 选择2018年8月至2020年8月于我院就诊的68例慢性牙周炎患者，一组采取SRP治疗（n=34），另一组采取Er:YAG联合SRP治疗（n=34），所有患者分别在治疗前、治疗后6周和治疗后12周检查牙周状况、龈沟液细胞因子水平和牙菌斑菌群比例，并在治疗后即刻进行疼痛程度评分，比较两组患者的相关指标。结果 治疗后所有患者牙周状况、龈沟液细胞因子水平和牙菌斑菌群占比均较治疗前有明显改善（P<.05）。治疗后6周，Er组患者SBI、PD、CAL较SRP组患者下降，治疗后12周，Er组患者PD、BOP、PI较SRP组降低（P<.05），其他指标在两组间差异无统计学意义（P>0.05）。治疗后12周，Er组患者IL-1β、IL-6水平低于SRP组，TGF-β、b-FGF水平高于SRP组，牙龈卟啉单胞菌和齿垢密螺旋体在菌群中的占比较同期SRP组明显降低，差异均具有统计学意义（P<.05）；而TC、TG、Ox-LDL、hs-CRP、IL-8和TNF-α水平在两组患者间的差异无统计学意义（P>0.05）。结论 Er:YAG激光联合SRP治疗慢性牙周炎较单一的SRP治疗能有效改善牙周状况，对牙龈卟啉单胞菌和齿垢密螺旋体有显著杀菌效果，临床疗效显著。 |

| Objective The aim of this study was to explore the clinical efficacy of Er:YAG laser combined with subgingival curettage and root planing (SRP) in the treatment of chronic periodontitis, and compare the periodontal indicators and other affecting factors. Methods The 68 patients with chronic periodontitis who were treated in our hospital from August 2018 to August 2020 were selected as the research objects. Patients in the SRP group were treated with SRP alone (n=34) and patients in the Er group were treated with Er:YAG combined with SRP (n=34), all patients were checked for periodontal status before treatment, 6 weeks after treatment, and 12 weeks after treatment. Blood lipid levels, gingival crevicular fluid cytokine levels, and the proportion of dental plaque flora were checked before treatment and 12 weeks after treatment. The pain degree will be scored immediately after treatment, and the relevant indicators of the two groups of patients will be compared. Results The periodontal status, blood lipid level, gingival crevicular fluid cytokine level and the proportion of dental plaque flora in all patients were significantly improved compared with those before treatment (P<0.05). Six weeks after treatment, SBI, PD, and CAL in the Er group were lower than those in the SRP group; and 12 weeks after treatment, PD, BOP and PI in the Er group were lower than those in the SRP group, the difference was statistically significant (P<0.05). Other indicators were no statistically significant difference between the two groups of patients (P>0.05). After 12 weeks of treatment, the levels of IL-1β and IL-6 in the Er group were lower than those in the SRP group, and the levels of TGF-β and b-FGF were higher than those in the SRP group; the proportion of Porphyromonas gingivalis and Treponema denticola in the Er group was significantly decreased, compared with SRP group during the same period, and the difference was statistically significant (P<0.05); the levels of TC, TG, Ox-LDL, hs-CRP, IL-8 and TNF-α were not significantly different between the two groups of patients (P>0.05). Conlusion Er:YAG laser combined with SRP in the treatment of chronic periodontitis can effectively improve the periodontal condition, reduce inflammation, and have a significant bactericidal effect on Porphyromonas gingivalis and Treponema denticola. It has a significant clinical effect. |
