      目的 探讨虚拟仿真标准化病人软件系统在口腔住院医师客观结构化临床考核的应用效果。方法 设计虚拟仿真标准化病人软件系统,应用于2020~2022年南京地区口腔住院医师规范化培训技能结业考核的病史采集与临床检查站点,将2020~2022年结业考核中该站点均分及考试用时与使用真人标准化病人的2019年结业考核该站点均分及用时进行比较,并分析考核难度、区分度,并设计调查问卷,探讨应用虚拟仿真标准化病人软件系统的考核效果及考官、考生对该种考核形式的认可度。结果 2019~2022年口腔住院医师规范化培训结业考核该站点的平均成绩及考试所用时间无显著差异(P>0.05),对2020~2022年应用虚拟仿真标准化病人软件系统后该站点考试成绩进行难度、区分度分析,统计结果均在合适范围。90%及以上考生及考官对VSP的应用持肯定态度,认为虚拟仿真技术的应用使口腔临床检查能力考核更加科学、严谨,考试方法和流程简单易懂,考核难度适中,与临床教学内容吻合,能够客观反应医学生临床检查能力水平。结论 将虚拟仿真标准化病人软件系统应用于口腔住院医师规范化培训临床技能考核,可以缓解病例资源的不足,同时也能客观检验学生临床检查能力,及时反馈教学问题,促进临床教学质量的提高,值得进一步研究和推广。
英文摘要:
      Objective To explore the application effect of a virtual standardized patients(VSP) software in objective structured clinical examination.Methods A virtual simulation standardized patient software was designed and applied to the objective structured clinical examination of standardized training skill completion for dental residents in Nanjing area from 2020 to 2022. The average score and test time of the medical history collection and clinical examination site were compared from 2019 to 2022. The difficulty and differentiation degree of assessment were analyzed. A questionnaire was designed to explore the assessment effect of virtual simulation standardized patients and the recognition degree of examiners and examinees to this form of assessment.Results There was no significant difference between the average score and the test time of the station(P>0.05). The difficulty and differentiation analysis of the test results of the station after application of the virtual simulation standardized patient software from 2020 to 2022 showed that the statistical results were in the appropriate range. More than 90% of the examinees and examiners held positive attitudes towards the application of virtual standardized patients, believing that the application of virtual simulation technology made the examination of oral clinical examination ability more scientific and rigorous. The examination methods and procedures were simple and easy to understand, and the examination difficulty was moderate, which was consistent with the clinical teaching content and can objectively reflect the level of clinical examination ability of medical students.Conclusion Application of the virtual standardized patient software in clinical skill assessment of standardized training for oral residents can alleviate the shortage of case resources, objectively test students' clinical examination ability, timely feedback teaching problems, and promote the quality of clinical teaching, which is worthy of further research and promotion.
