| 目的 研究颏联合牙槽外微种植钉压低下前牙的有效性。方法 选择因下前牙伸长导致的前牙III度深覆18例（男11例，女7例），在颏联合平行于牙长轴植入微种植钉2枚，结合远中延伸力臂施加压低力，必要时施加远中向拉力控制前牙转矩。治疗前、后CBCT影像体素重叠，分析下前牙的位置变化。采用Graphpad Prism 6.0软件包对下前牙压低前、后数据进行配对t检验，P<0.05为有显著性差异。结果 所有病例治疗结束后下前牙均有效压低，L1-MP、LCR-MP明显减小，spee’s曲线整平（P<0.05），前牙覆明显改善，IMPA增加但无统计学差异，牙根长度未见明显改变。结论 颏联合牙槽外微种植钉可提供有效力学系统，实现下前牙的绝对压低。 |

| Objective To investigate the effectiveness of the lower anterior teeth intrusion with miniscrews implanted in extra-alveolar mental symphysis.Methods 18 cases (11 males and 7 females) of anterior deep overbite (class Ⅲ) caused by extrusion of the lower anterior teeth were selected,and two miniscrews parallel to the tooth axis were implanted in extra-alveolar mental symphysis.The intrusion force was applied from the distal extension arm attached to the miniscrew.If necessary,the distal force was applied concurrently to control the torque of the anterior teeth.Analyzing the position changes of the lower anterior teeth was based on the voxel overlap of pre and post-treatment CBCT imaging data.Paired t-test was conducted on the data before and after treatment with the Graphpad Prism 6.0 software package,and P<0.05 was considered as a significant difference.Results All the lower anterior teeth were effectively intruded,L1-MP and LCR-MP were significantly reduced,the spee's curve was flattened (P<0.05),and the anterior deep overbite was improved significantly.IMPA was increased without statistically difference,the root length did not show any significant change either.Conclusion The miniscrews implanted in extra-alveolar mental symphysis can provide effective force system to achieve absolute intrusion of the lower anterior teeth. |
