目的 探讨Nd:YAG激光和Er:YAG激光治疗牙本质敏感(Dentin Hypersensitivity,DH)的效果。方法 收治佛山市第一人民医院2021年3月至2022年12月期间就诊的牙本质敏感93例患者共259颗牙,患者均在受到冷、酸、甜及刷牙等刺激后存在酸痛感,诊断为DH。随机分为3组,分别给予常规脱敏剂治疗组(31例患者,86颗患牙)、Nd:YAG激光治疗组(31例患者,90颗患牙)、Er:YAG激光治疗组(31例患者,83颗患牙)3种方法,其中激光组尝试改变激光照射参数及时间,寻求最好的治疗参数。对比3种脱敏方法的即刻效果、1周效果、1个月效果及3个月效果。结果 治疗前3组对冷空气及机械刺激反应的VAS评分无明显差异(P>0.05);治疗后各个时间段Nd:YAG激光组和Er:YAG激光组对冷空气刺激反应均明显轻于对照组(P<0.05)。治疗后各个时间段Nd:YAG激光组和Er:YAG激光组对机械刺激反应均明显轻于对照组(P<0.05)。3组经治疗后均获得一定疗效,Nd:YAG激光组和Er:YAG激光组的效果好于常规组,但差异无统计学意义(P>0.05)。结论 Nd:YAG激光、Er:YAG激光治疗DH与常规脱敏方法相比,明显降低患者的冷空气和机械刺激反应,在一定程度上提升了治疗效果。 |

Objective To investigate the effect of Nd:YAG laser and Er:YAG laser on dentin hypersensitivity (DH).Methods A total of 259 teeth from 93 patients with dentin sensitivity were admitted to First People's Hospital of Fochan from March 2021 to December 2022.The patients all experienced soreness and pain after being stimulated by cold,sour,sweet,and brushing their teeth,and were diagnosed as DH.The patients were randomly divided into three groups and treated with three methods: conventional desensitization treatment group (86 affected teeth in 31 patients),Nd:YAG laser treatment group (90 affected teeth in 31 patients) and Er:YAG laser treatment group (83 affected teeth in 31 patients),among which the laser group tried to change the laser irradiation parameters and time to seek the best treatment parameters.The immediate effect,one-week effect,one-month effect,and three-month effect of the three desensitization methods were compared.Results Before treatment,there were no significant differences in VAS scores of the three groups in response to cold air and mechanical stimulation (P> 0.05).The response of Nd: YAG laser group and Er: YAG laser group to cold air stimulation was significantly lighter than that of control group at all time periods after treatment (P<0.05).The response of Nd:YAG laser group and Er:YAG laser group to mechanical stimulation was significantly less severe than that of control group at all time periods after treatment (P<0.05).After treatment,all three groups obtained certain curative effect,Nd:YAG laser group and Er:YAG laser group were better than the conventional group,but the difference was not statistically significant (P>0.05).Conclusion Nd:YAG laser and Er:YAG laser can significantly reduce cold air and mechanical stimulation response in the treatment of DH compared with conventional desensitization methods,and improve the therapeutic effect to a certain extent. |
