目的 评价外科牵引治疗前牙区复杂冠根折的疗效。方法 选择2021年9月至2023年9月就诊于上海交通大学医学院附属第九人民医院口腔综合科牙外伤专科的复杂冠根折患者20例,应用外科牵引方式保留患牙,检查记录手术牙牙齿松动度、牙周探诊深度、牙龈指数,术前和牵引术后即刻,术后3个月和术后 1年拍摄 CBCT,测量术后唇侧牙槽嵴顶至根尖高度变化,使用 SPSS 23.0软件进行统计学分析,评价其治疗效果。结果 20例患者平均牵引长度 3.575 mm,1年成功率为 100%。患者对患牙美学与咀嚼功能满意,影像学检查显示手术牙均未出现明显牙根外吸收,术后即刻、术后3个月和1年的牙周探诊深度和牙龈指数的差异无明显统计学意义(P>0.05),唇侧牙槽嵴顶至根尖高度变化无明显统计学差异(P>0.05)。结论 通过正确把握手术适应证和规范操作,外科牵引应用于前牙区复杂冠根折的治疗是一种可靠的方法,短期临床效果良好,本研究为复杂冠根折的患牙保存治疗提供了新的思路。 |

Objective To assess the therapeutic efficacy of surgical extrusion in the treatment of complex crown-root-fractured in the anterior tooth region. Methods Twenty cases of patients with complex crown-root fractures who received treatment at the Dental Trauma Specialty of the Comprehensive Dentistry Department of the Ninth People's Hospital Affiliated to Shanghai Jiao Tong University School of Medicine from September 2021 to September 2023 were selected. The affected teeth were retained through surgical extrusion. The tooth mobility, periodontal probing depth, and gingival index of the surgical teeth were examined and recorded. CBCT was taken before the operation, immediately after the extrusion, 3 months, and 1 year after the operation. The change in the length of the buccal alveolar crest was measured. Statistical analysis was conducted using SPSS 23.0 software to evaluate the therapeutic effect. Results The average extrusion length of the 20 patients was 3.575 mm, and the success rate after one year was 100%. The patients were satisfied with the aesthetics and chewing function. The imaging examination showed no obvious external root resorption in the surgical teeth. There was no significant statistical difference in the periodontal probing depth and gingival index (P>0.05). There was no significant statistical difference in the change of the height from the buccal alveolar crest to the root tip immediately after the treatment, 3 months, and 1 year after the treatment (P>0.05). Conclusion In this study, by accurately grasping the surgical indications and following standardized procedures, the application of surgical extrusion in the treatment of complex crown-root fractured anterior teeth is a reliable approach with favorable short-term clinical outcomes, providing a novel idea for the preservation treatment of complex crown-root fractures. |
