目的 探究铒激光联合血运重建对牙髓坏死的年轻恒牙根管壁厚度及炎性因子水平的影响。方法 选取本院2020年6月至2023年6月期间治疗的80例年轻恒牙牙髓坏死患者,将其分为观察组(n=40)和对照组(n=40),对照组行超声荡洗+氢氧化钙糊剂消毒根管,观察组行铒激光消毒根管,两组均行血运重建;比较两组牙冠根比和牙根管壁厚度,牙周指数及血清白细胞介素-6(IL-6),白细胞介素-8(IL-8)和肿瘤坏死因子-α(TNF-α)水平。结果 观察组治疗后的总有效率(92.50%)显著高于对照组(75.00%),差异具有统计学意义(P<0.05)。治疗后两组的牙冠根比和牙根管壁厚度均明显升高(P<0.05),其中观察组显著高于对照组(P<0.05)。治疗后两组的龈沟出血指数(SBI)、菌斑指数(PLI)、牙周探诊深度(PD)、IL-6,IL-8 和 TNF-α显著降低(P<0.05),观察组相比对照组更明显(P<0.05)。观察组与对照组不良反应比较无差异(P>0.05)。结论 铒激光联合血运重建可以增加根管壁厚度,降低机体炎性因子IL-6,IL-8和TNF-α水平,提高年轻恒牙牙髓坏死患者疗效。 |

Objective To investigate the effect of erbium laser combined with revascularization on root canal wall thickness and inflammatory factor levels in young permanent teeth with pulp necrosis. Methods A total of 80 patients with pulp necrosis in young permanent teeth who received treatment in our hospital from June 2020 to June 2023 were included in this study. Patients were randomly assigned to the observation group (n=40) and the control group (n=40). The control group underwent ultrasonic irrigation combined with calcium hydroxide paste for root canal disinfection, while the observation group received erbium laser disinfection. Both groups subsequently underwent revascularization. The crown-to-root ratio, root canal wall thickness, periodontal indices, and serum levels of interleukin-6 (IL-6), interleukin-8 (IL-8), and tumor necrosis factor-alpha (TNF-α) were compared between the two groups. Results The total effective rate in the observation group (92.50%) was significantly higher than in the control group (75.00%) (P<0.05). After treatment, both groups exhibited a significant increase in the crown-to-root ratio and root canal wall thickness (P<0.05), with the observation group showing significantly greater improvements compared to the control group (P<0.05). Additionally, the gingival sulcus bleeding index (SBI), plaque index (PLI), periodontal probing depth (PD), and levels of IL-6, IL-8, and TNF-α were significantly reduced in both groups after treatment (P<0.05), with the reductions being more pronounced in the observation group (P<0.05). No significant differences in adverse reactions were observed between the two groups (P>0.05). Conclusion Erbium laser combined with revascularization effectively increases root canal wall thickness, reduces inflammatory factor levels (IL-6, IL-8, TNF-α), and improves clinical outcomes in young permanent teeth with pulp necrosis. |
