Objective To evaluate the clinical efficacy of using implant anchorage for controlled molar intrusion and micro-osteoperforations (MOPs) to accelerate mesial movement of posterior teeth in the multidisciplinary treatment of complex cases involving orthodontics, prosthodontics, and periodontics. Method A case of a maxillary canine transposed to the labial side of the ipsilateral central incisor was examined through intraoral and radiographic assessments. A multidisciplinary approach integrating orthodontic, prosthetic, and periodontal treatment was employed. The treatment plan involved the use of temporary anchorage devices (TADs) for controlled molar intrusion, MOPs to facilitate accelerated tooth movement, and restorative techniques to achieve functional and esthetic rehabilitation. Results The maxillary canine successfully replaced the ipsilateral central incisor, and esthetic enhancement was achieved through veneer reshaping. The use of TADs effectively controlled molar intrusion, while MOPs significantly accelerated posterior tooth mesialization. Conclusion Adult orthodontic patients with complex malocclusions and compromised dental conditions can benefit from individualized treatment strategies. Replacing an unsalvageable central incisor with a canine can yield satisfactory functional and esthetic outcomes. Additionally, TADs provide efficient molar intrusion, and MOPs can expedite tooth movement, reducing overall treatment duration and optimizing time efficiency. This case demonstrates the successful integration of orthodontic, prosthetic, and periodontal disciplines in managing complex dental anomalies.
