目的 评价选择性激光熔化(selective laser melting,SLM)成型技术打印单翼粘接桥修复个别前牙缺失的临床效果。方法 选取个别前牙缺失患者32人,基牙预备后,运用光学扫描获取数字化模型,在CAD软件中设计单翼粘接桥基底,通过SLM设备3D打印桥基底;然后在树脂模型上完成饰瓷堆塑,再制作义齿辅助定位导板,临床完成粘接后6个月、12个月和24个月随访;根据改良美国公共卫生署(United Statespublic health service,USPHS)标准评价修复效果,采用视觉模拟评分法(visual analog scale,VAS)评价患者对修复效果的满意度。治疗24个月后,通过基牙和邻近天然牙比较牙周指数[牙龈指数(GI)、龈沟探诊深度(SPD)、龈沟出血指数(SBI)、菌斑指数(PLI)]。结果 随访期内32例患者中有1例修复体12个月复查内发生脱粘接,USPHS标准评价修复效果除6例颜色匹配评级为B级,其余各项指标为A级。各随访期内32例患者满意度评分分别为(9.4±0.5)、(9.5±0.3)和(9.6±0.2)分。24个月复诊时粘接桥基牙GI、SPD、SBI及PLI值与邻近天然牙的相应牙周指数的差异无统计学意义(P>0.05)。结论 SLM 3D打印单翼粘接桥应用于个别前牙缺失的治疗具有良好的临床效果。 |

Objective To evaluate the clinical performance of single-wing adhesive bridges fabricated using Selective Laser Melting (SLM) technology in the restoration of individual anterior tooth loss, and to explore key clinical procedures. Methods Thirty-two patients with individual anterior teeth loss were enrolled. After abutment tooth preparation, digital models were obtained through optical scanning. The single-wing adhesive bridge framework was designed using CAD software and fabricated via SLM-based 3D printing. Porcelain veneering was completed on a resin model, followed by fabrication of an auxiliary positioning guide. Clinical bonding was performed, and follow-ups were conducted at 6-, 12-, and 24-month post-treatment. Restoration outcomes were evaluated using the modified United States Public Health Service (USPHS) criteria, while patient satisfaction was assessed using a Visual Analog Scale (VAS). After 24 months, periodontal indices, including Gingival Index (GI), Sulcus Probing Depth (SPD), Sulcus Bleeding Index (SBI), and Plaque Index (PLI), were compared between abutment teeth and adjacent natural teeth. Results During follow-up, one restoration experienced debonding at the 12- month evaluation. According to the USPHS criteria, 6 cases were rated as grade B for color matching, while all other parameters were graded as A. Patient satisfaction scores at 6, 12, and 24 months were 9.4 ±0.5, 9.5 ±0.3, and 9.6 ±0.2, respectively. At the 24-month follow-up, no significant differences were observed in GI, SPD, SBI, or PLI between abutment teeth and adjacent natural teeth (P>0.05). Conclusion SLM 3D-printed single-wing adhesive bridges exhibit excellent clinical efficacy for the restoration of individual anterior tooth loss and are a viable option for minimally invasive treatment. |
