目的 在3D打印牙模上评价四手联合操作对根管治疗效果的影响。方法 选用20颗下颌第一磨牙(4根管)的3D打印牙模型,随机分为两组,每组10个。所有操作在仿真头模内进行,并配合使用口腔显微镜。对照组由医师在传统护理配合下完成治疗,实验组由同一医师在四手联合操作下完成治疗。比较两组根管治疗时间(包括上橡皮障、开髓、根管预备、根管充填、树脂修复、治疗总时间);利用X线片及CBCT评价两组根管填充的质量(包括根充物距解剖根尖孔位置及根充密合度)。应用SPSS 29.0统计学软件对两组各治疗时间进行统计学分析。结果 实验组治疗的总操作时间(42.70 min)短于对照组(64.40 min),两组总治疗时间的差异具有统计学意义(P<0.01)。两组充填物距根尖距离均在0.5~2 mm范围内,充填物与根管壁间无空隙;且CBCT横断面显示充填物均匀致密,严密封闭整个根管系统。两组根管充填质量均符合恰填标准。结论 四手联合操作应用于显微根管治疗中,可缩短治疗时间,减短患者的就诊时间,提高工作效率及减少医生的身体疲劳。 |

Objective To evaluate the impact of four-handed dentistry on the efficiency and effectiveness of root canal treatment using 3D-printed mandibular molar models. Method A total of 20 3D-printed mandibular first molars (each with four canals) were randomly divided into two groups (n=10 per group). All procedures were performed inside a simulated dental head phantom and under an operating dental microscope. In the control group, the dentist completed the treatment using traditional single-handed assistance, while in the experimental group, the same dentist performed the procedure with the support of a four-handed dental team. The total treatment time and the duration of each step, including rubber dam placement, access cavity preparation, root canal instrumentation, obturation, and resin restoration, were recorded and compared. The quality of root canal filling was assessed using periapical radiographs and CBCT scans, evaluating both the distance between the filling material and the apical foramen and the adaptation of the filling to the canal walls. Statistical analysis was performed using SPSS 29.0. Results The total treatment time in the experimental group (42.70 minutes) was significantly shorter than that of the control group (64.40 minutes) (P<0.01). In both groups, the root canal filling material was located within 0.5-2 mm of the anatomical apex, with no gaps observed between the filling and canal walls. CBCT cross-sections showed dense and uniform obturation in all samples, meeting the criteria for acceptable root canal filling quality. Conclusion The application of four-handed dentistry in microscopic root canal treatment can significantly reduce treatment time, shorten patient chairside duration, enhance clinician efficiency, and reduce operator fatigue. |
