      目的 探讨青少年骨性Ⅱ类不同垂直骨面型错 畸形患者利用两种功能矫治器治疗前后的软硬组织变化。方法 收集44例生长发育期骨性Ⅱ类错 畸形患者,其中22例均角偏高角及高角(MP>34.3°),患者佩戴固定式下颌前导矫治器,22例均角偏低角及低角(MP≤34.3°)患者选用 Sagittla-guidance Twin-block(SGTB)矫治器完成治疗。治疗前后拍摄X线片,并测量治疗前后软硬组织的变化。结果 22例均角偏高角及高角骨性Ⅱ类错 畸形患者治疗后 SNB、Ptm-A、CO-GO,GO-PO、ANS-Me、L1-MP、L6-MP、Li-TVL、Pog’-TVL增大,ANB、U1-SN、Ls-TVL减小,差异均有统计学意义(P<0.05),其余指标均无统计学意义(P>0.05)。利用 SGTB完成治疗的 22例均角偏低角及低角骨性Ⅱ类患者,治疗后,SNB、Ptm-A、CO-GO,GO-PO、MP-SN、ANS-Me、U6-PP、L6-MP、Li-TVL、Pog’-TVL增大,ANB、U1-SN 减小,差异有统计学意义(P<0.05)。结论 青少年骨性Ⅱ类不同垂直骨面型功能矫治均可促进下颌骨发育,改善患者侧貌。其中均角偏高角及高角患者采用粘接固定式下颌前导矫治器治疗,垂直向控制效果好,扩大了骨性Ⅱ类功能矫治适应证。SGTB可以有效打开均角偏低角及低角患者咬合,改善面下1/3不足。

      Objective To investigate the changes in soft and hard tissues before and after treatment with two different functional appliances in adolescents with skeletal Class II malocclusion and varying vertical skeletal patterns. Methods A total of 44 adolescent patients with skeletal Class II malocclusion were recruited. Of these, 22 patients with high or average-high mandibular plane angles (MP > 34.3°) were treated with a bonded fixed mandibular protraction appliance, while 22 patients with low or average-low MP (MP ≤ 34.3°) received treatment with sagittal-guidance Twinblock (SGTB). X-ray images were taken and measurements were conducted before and after treatment to analyze changes in soft and hard tissues using different functional orthoduntic appliances for adolescents with skeletal Class II malocclusion and different vertical plane types. Results In the high-angle group treated with the fixed mandibular protraction appliance, SNB, Ptm-A, CO-GO, GO-PO, ANS-Me, L1-MP, L6-MP, Li-TVL, and Pog’-TVL significantly increased, while ANB, U1-SN, and Ls-TVL significantly decreased (P < 0.05). The remaining indicators were not statistically significant (P < 0.05). In the low-angle group treated with SGTB, significant increases were observed in SNB, Ptm-A, CO-GO, GOPO, MP-SN, ANS-Me, U6-PP, L6-MP, Li-TVL, and Pog'-TVL, with decreases in ANB and U1-SN (P < 0.05). The remaining indicators were not statistically significant (P < 0.05). Conclusion Both types of functional appliances were effective in promoting mandibular growth and improving facial profiles in adolescents with skeletal Class II malocclusion. The fixed mandibular protraction appliance was more effective in vertical control for high-angle patients, broadening its clinical indications. SGTB effectively facilitated bite opening and improved lower facial deficiencies in low-angle cases.
