      目的 分析龈沟液中高迁移率族蛋白B1(HMGB1)和趋化素(Chemerin)表达水平与种植体周围炎发生及预后的相关性。方法 将108例种植体周炎患者设为观察组(依预后分预后良好组49例、不良组59例),同期108例种植体周围健康者为对照组,用酶联免疫吸附法测龈沟液中HMGB1和Chemerin水平,Pearson/Spearman分析其与多项指标相关性,ROC曲线分析其对预后不良预测价值。结果 观察组患者龈沟液中HMGB1和Chemerin水平高于对照组(P<0.05);预后不良组高于预后良好组(P<0.05);其与牙周炎分期分级等多指标呈正相关(P<0.05)。ROC结果显示,两者联合预测种植体周炎患者预后不良的AUC为0.865,高于单独预测(P<0.05)。结论 种植体周炎患者龈沟液内HMGB1和Chemerin表达水平升高,且与种植体周围炎发生和预后具有一定相关性。

      Objective To analyze the correlation between the expression levels of high-mobility group box 1(HMGB1) and chemerin in gingival crevicular fluid(GCF) and the occurrence and prognosis of peri-implantitis. Methods A total of 108 patients with peri-implantitis were included as the observation group and further divided into a good-prognosis group(n=49) and a poor-prognosis group(n=59) according to clinical follow-up outcomes. Another 108 individuals with healthy peri-implant tissues during the same period were included as the control group. The levels of HMGB1 and chemerin in GCF were measured by enzyme-linked immunosorbent assay(ELISA). Pearson/Spearman correlation analyses were performed to assess their relationships with various clinical indicators, including periodontitis stage and grade, gingival bleeding index, probing depth, GCF volume, and bone resorption. Receiver operating characteristic(ROC) curves were used to evaluate the predictive value for poor prognosis. Results The levels of HMGB1 and Chemerin in GCF were significantly higher in the observation group than in the control group(P<0.05), and higher in the poor-prognosis group than in the good-prognosis group(P<0.05). Both biomarkers were positively correlated with several clinical indicators, including periodontitis staging and grading(P<0.05). ROC curve analysis showed that the combined detection of HMGB1 and Chemerin yielded an AUC of 0.865, which was higher than that of either marker alone(P<0.05). Conclusion HMGB1 and Chemerin levels in gingival crevicular fluid are elevated in patients with peri-implantitis and are associated with the occurrence and prognosis of the disease.
