      目的 比较机用镍钛MTwo-R和M3-RT系统对拆除中度弯曲根管充填物的能力,为临床应用提供参考。方法 收集人离体下颌第一恒磨牙中度弯曲的近中根管28个,以M3-PRO镍钛锉进行根管疏通和成形。所有根管进行冷侧压充填后,分别使用MTwo-R和M3-RT再治疗锉拆除根管充填物。拆除根管充填物前后均使用micro-CT进行扫描。记录拆除根管充填物的时间、清除率、根尖偏移量和再治疗锉形变等情况。采用独立样本t-检验和卡方检验比较组间差异。结果 1)MTwo-R组根管充填物拆除的用时(150.36±65.60) s长于M3-RT组(86.00±21.47) s;2组差异有统计学意义(P=0.000)。2)MTwo-R组根管充填物的清除率(71.58±10.45)%低于M3-RT组(88.48±6.39)%;但2组差异无统计学意义(P=0.141)。3)距离根尖3 mm处,MTwo-R组的根尖偏移量较大(0.1890±0.1669),M3-RT组偏移量较小(0.0946±0.0405);2组差异有统计学意义(P<0.05)。但2组在距离根尖5 mm和1 mm处的偏移量均无显著性差异(P>0.05)。4)MTwo-R组再治疗锉的形变率较高(42.85%),M3-RT组形变率较低(4.76%),2组差异有统计学意义(P=0.002)。结论 相比较MTwo-R,M3-RT系统拆除根管充填物所需时间较短,根管充填物的清除率较高,且器械使用寿命和安全系数更高。在根尖偏移量差异不大的情况下,M3-RT系统更适用于中度弯曲根管的充填物拆除。

      Objective Comparing the pros and cons of MTwo-R and M3-RT re-treatment files in root canal fillings removal in terms to provide references for clinical application. Methods After Twenty-eight mesial root canal with moderate curvature of extracted first mandibular permanent molar were collected, M3-PRO preparation files were used for root canal glide path and shaping preparation. Then, all the samples were underwent root canal obturation. MTwo-R and M3-RT re-treatment systems were used to remove the root canal fillings. Micro-CT scan was used before and after the removal of root canal fillings. All data including time required and clearance rate of root canal fillings removal, apical transportation and deformation of re-treatment files. Independent sample t-test and chi-square test were used to compare the differences between groups. Results 1) Root canal re-treatment time: The MTwo-R group required longer time in root canal fillings removal(150.36±65.60) s, while the M3-RT group required lesser time in root canal fillings removal(86.00±21.47) s. There was significant difference between Mwo-R group and M3-RT group(P=0.000). 2) Clearance rate of root canal fillings: The MTwo-R group exhibited a lower clearance rate(71.58±10.45) %, whereas the M3-RT group exhibited a higher clearance rate(88.48±6.39) %. However, the difference between the two groups was not statistically significant(P=0.141).3) Apical canal transportation: At 5 mm from the apex, MTwo-R group exhibited a lesser transportation(0.0759±0.0474), while M3-RT group exhibited a greater transportation(0.1285±0.0874). At 3 mm from the apex, MTwo-R group exhibited a greater transportation(0.1890±0.1669), while M3-RT group exhibited a lesser transportation(0.0946±0.0405). At 1 mm from the apex, MTwo-R group exhibited a lesser transportation(0.1234±0.0478), while M3-RT group exhibited a greater transportation(0.1570±0.0717). There were no significant differences in apical transportation between the two groups at 5 mm, 3 mm, and 1 mm from the apex(P>0.05). 4) Instrument defect: MTwo-R group exhibited a higher deformation rate(42.85%), while M3-RT group exhibited a lower deformation rate(4.76%). The difference between the two groups was statistically significant(P=0.002). Conclusion M3-RT system not only showed higher efficiency and clearance rate in root canal fillings removal, as well as the lifespan and safety coefficient when compared with MTwo-R system. Thus, M3-RT system is more suitable for root canal fillings removal in moderate curved root canals when apical transportation rate is not significant.
