| 目的 探讨环钻取骨同期植入种植体修复牙槽骨三维骨量不足的短期临床效果，为该类患者的牙槽骨三维骨缺损修复提供可行性方案。方法 收集2015年行环钻取骨同期种植体植入术的牙槽骨三维骨量不足患者共6例；术前CBCT评估骨缺损；术中于颏部、外斜嵴或硬腭取环状骨块，同期植入种植体并行GBR术；术后6个月CBCT复查，完成二期手术及种植义齿修复。观察骨增量、种植体稳定性及患者满意度。结果 6例患者手术顺利，创口Ⅰ期愈合。术后CBCT示牙槽嵴丰满，骨增量充足；6个月后二期手术时种植体稳固，未见松动及明显骨吸收。完成修复后，通过影像学检查未见种植体周围炎及显著边缘骨吸收，患者均表示满意。结论 环钻取骨同期植入种植体可在短期内有效恢复牙槽骨三维骨量并实现种植体短期稳定，具备良好的临床应用前景。 |

| Objective To investigate the short-term clinical effect of simultaneous trephine bone extraction and implant placement for the restoration of alveolar bone with insufficient three-dimensional bone volume, and to provide a feasible approach for patients with such conditions. Methods Six patients with 3D alveolar bone deficiency who underwent trephination and simultaneous implant placement in 2015 were collected. Preoperative CBCT was used to evaluate bone defects. Annular bone blocks were taken from the chin, external oblique ridge, or hard palate, followed by simultaneous implant placement and guided bone regeneration(GBR). Six months after the operation, CBCT examination was conducted, followed by the second-stage implant surgery and final prosthetic restoration. Bone augmentation outcomes, implant stability, and patient satisfaction were subsequently assessed. Results The operation was successful in all six patients, and the wounds healed by primary intention. Postoperative CBCT showed that the alveolar ridge was full and the bone increment was sufficient. Six months after surgery, the implants were stable, with no loosening or bone resorption observed. Radiographic follow-up revealed no peri-implantitis or marginal bone loss, and all patients showed satisfactory outcomes. Conclusions The technique of simultaneous trephine bone extraction and implant placement can effectively restore alveolar bone with insufficient three-dimensional bone volume in the short term, providing stable implant outcomes and demonstrating reliable clinical application value. |
