| 目的 探究龈沟液微小RNA-145-5p(miR-145-5p)与微小RNA-140-3p(miR-140-3p)联合检测在慢性牙周炎(chronic1 periodontitis,CP)严重程度及预后评估中的价值。方法 选择作者所在医院诊治的147例CP患者,根据炎症程度分为轻度(n=48)、中度(n=57)和重度3组(n=42),另选85例无CP受试者为对照组。荧光定量PCR(qRT-PCR)检测miR-145-5p、miR-140-3p表达水平,检测患者5项牙周临床指标。多因素Logistic回归分析CP患者预后不良的影响因素;Pearson和Spearman相关性分析miR-145-5p、miR-140-3p水平与各影响因素的相关性;ROC曲线分析其对CP患者预后不良的诊断价值,Z检验比较AUC的差异。结果 5项牙周临床指标以及miR-145-5p、miR-140-3p水平在不同程度CP受试者与对照组之间存在差异(P<0.05);其中预后不良组高于预后良好组(P<0.05);miR-145-5p和miR-140-3p水平与5项牙周临床指标之间存在一定相关性(P<0.05);miR-145-5p、miR-140-3p水平升高是CP患者预后不良的独立危险因素(P<0.05);其单独诊断CP患者预后不良的AUC分别为0.801、0.806,联合诊断的AUC为0.935,优于各自单独诊断(均P<0.05)。结论 miR-145-5p和miR-140-3p水平随着CP严重程度不断增加,是CP患者预后不良的影响因素,两者联合诊断CP患者预后不良具有一定的价值。 |

| Objective To investigate the value of combined detection of microRNA-145-5p (miR-145-5p) and microRNA-140-3p (miR-140-3p) in gingival crevicular fluid for assessing the severity and prognosis of chronic periodontitis (CP). Methods A total of 147 patients with CP were enrolled and classified into mild (n= 48), moderate (n=57), and severe (n=42) groups according to disease severity. In addition, 85 subjects without CP were included as the control group. The expression levels of miR-145-5p and miR-140-3p were measured by quantitative real-time PCR (qRT-PCR), and five periodontal clinical parameters were assessed. Multivariate logistic regression analysis was performed to identify factors associated with poor prognosis in patients with CP. Pearson and Spearman correlation analyses were used to examine the correlations between miR-145-5p and miR-140-3p levels and the relevant factors. Receiver operating characteristic (ROC) curve analysis was performed to evaluate their diagnostic value for poor prognosis in patients with CP, and Z tests were used to compare differences in the area under the curve (AUC). Results Significant differences were observed in the five periodontal clinical parameters and in miR-145-5p and miR-140-3p levels among patients with different severities of CP and the control group (P<0.05). These indicators were all higher in the poor prognosis group than in the good prognosis group (P<0.05). Both miR-145-5p and miR-140-3p levels were correlated with the five periodontal clinical parameters (P<0.05). Elevated miR-145-5p and miR-140-3p levels were independent risk factors for poor prognosis in patients with CP (P<0.05). The AUCs of miR-145-5p and miR-140-3p alone for predicting poor prognosis in patients with CP were 0.801 and 0.806, respectively, while the combined AUC was 0.935, which was superior to either marker alone (both P<0.05). Conclusion The levels of miR-145-5p and miR-140-3p increased with increasing severity of CP and were associated with poor prognosis in patients with CP. Their combined detection has value in assessing poor prognosis in patients with CP. |
