      目的 探究慢性牙周炎(CP)患者龈沟液微小RNA-218-5p(miR-218-5p)、微小RNA-335-5p(miR-335-5p)水平及与疾病严重程度的关系。方法 选择2022年3月至2024年3月就诊的121例CP患者记为CP组,根据疾病严重程度分为轻度组(n=41)、中度组(n=54)和重度组(n=26)。qRT-PCR检测miR-218-5p、miR-335-5p表达水平,Pearson相关性分析相关性;多因素Logistic回归分析影响因素;ROC曲线分析诊断价值。结果 CP组miR-218-5p、miR-335-5p水平低于对照组(P<0.05);随着CP严重程度增加,炎症因子(IL-6、CRP、TNF-α)、牙周临床指标(SBI、GI、AL、PD、ABL)水平升高,而miR-218-5p、miR-335-5p水平下降(P<0.05);miR-218-5p、miR-335-5p水平与CP患者炎症因子、牙周临床指标均呈负相关(P<0.05);miR-218-5p、miR-335-5p联合诊断中度及重度CP发生的AUC分别为0.916、0.920,优于各自单独诊断(0.802、0.771和0.802、0.771)(P<0.05)。结论 miR-218-5p、miR-335-5p在CP患者龈沟液中水平较低,并随着疾病严重程度的增加逐渐降低,两者联合诊断中度及重度CP发生具有一定的价值,可作为CP治疗的潜在靶点。

      Objective To investigate the levels of microRNA-218-5p (miR-218-5p) and microRNA-335-5p (miR-335-5p) in gingival crevicular fluid of patients with chronic periodontitis (CP) and their relationship with disease severity. Methods A total of 121 patients with CP were enrolled and further classified into a mild group (n=41), a moderate group (n=54), and a severe group (n=26) according to disease severity. The expression levels of miR-218-5p and miR-335-5p were measured by quantitative real-time PCR (qRT-PCR). Pearson correlation analysis was used to assess correlations, multivariate logistic regression analysis was performed to identify influencing factors, and receiver operating characteristic (ROC) curve analysis was used to evaluate diagnostic value. Results The levels of miR-218-5p and miR-335-5p were lower in the CP group than in the control group (P<0.05). With increasing severity of CP, the levels of inflammatory markers (IL-6, CRP, and TNF-α) and periodontal clinical indicators (SBI, GI, AL, PD, and ABL) increased, whereas the levels of miR-218-5p and miR-335-5p decreased (P<0.05). The levels of miR-218-5p and miR-335-5p were negatively correlated with inflammatory markers and periodontal clinical indicators in patients with CP (P<0.05). For the diagnosis of moderate and severe CP, the AUCs of the combined detection of miR-218-5p and miR-335-5p were 0.916 and 0.920, respectively, which were superior to those of either marker alone (P<0.05). Conclusion The levels of miR-218-5p and miR-335-5p are reduced in the gingival crevicular fluid of patients with CP and decrease progressively with increasing disease severity. Their combined detection has value in the diagnosis of moderate and severe CP and may provide reference for the assessment of disease severity.
