| 目的 探究隐形矫治器和直丝弓固定矫治器治疗对深覆(牙合)患者软硬组织变化的影响。方法 按照治疗方式不同，将本院2022年1月至2023年5月收治的159例深覆(牙合)患者分为参照组（79例，直丝弓固定矫治器治疗）和试验组（80例，隐形矫治器治疗）；比较两组患者治疗前后龈沟液炎症指标（肿瘤坏死因子-a TNF-α、白细胞介素-8 IL-8、白细胞介素-1β IL-1β）、软组织指标（颏前点及上下唇突点切线与眼耳平面夹角Z角、下唇突度LLP、上唇突点至E线垂直距离Ulip-E）、硬组织指标（下中切牙长轴与下颌平面的后上交角L1-MP、上齿槽座角SNA、上中切牙长轴与NA连线的交角U1-NA、上下齿槽座角ANB）及牙龈萎缩、牙骨质粘连、牙根吸收等并发症发生情况。结果 治疗后，两组TNF-α、IL-8、IL-1β及Z角、L1-MP均升高，LLP、U1-NA、ANB均降低；其中试验组TNF-α、IL-8、IL-1β、LLP、U1-NA、ANB低于参照组，Z角、L1-MP高于参照组（P<0.05）；两组Ulip-E、SNA差异无统计学意义（P>0.05）；试验组并发症总发生率（2.5%）低于参照组（11.39%）(P<0.05)。结论 隐形矫治器治疗深覆(牙合)可改善患者软硬组织关系，减轻炎症反应，并发症发生率更低。 |

| Objective To explore the effect of clear aligner and straight wire appliance on soft and hard tissue changes in deep bite patients. Methods From January 2022 to May 2023, 159 patients with deep overbite at our hospital were divided into a control group(79 cases, treated with straight arch fixed appliances) and an experimental group(80 cases, treated with clear aligners). The study compared pre-and post-treatment gingival inflammation markers(TNF-α, IL-8, IL-1β), soft tissue parameters(Z angle, LLP, Ulip-E), and hard tissue indicators(L1-MP, ANB) in both groups. Complications such as gingival recession, periodontal adhesion, and root resorption were also evaluated. Results showed increased TNF-α, IL-8, IL-1β, Z angle, and L1-MP levels in both groups after treatment, while LLP, U1-NA, and ANB decreased. The experimental group exhibited lower TNF-α, IL-8, IL-1β, LLP, U1-NA, and ANB levels compared to the control group, with higher Z angle and L1-MP(P<0.05). No significant differences were found in Ulip-E and SNA levels between the groups(P>0.05). The experimental group also demonstrated a lower overall complication rate(P<0.05). Conclusion Clear aligner can improve the relationship between soft and hard tissue, reduce inflammation and lower complication rate in patients with deep overbite. |
