| 目的 探讨单支锉系统在后牙一次性根管治疗中的应用价值。方法 选取153例拟行后牙一次性根管治疗的患者，以随机数表分为W组、M组和P组，每组各51例。W组采用单支锉系统Wave one Gold进行一次性根管治疗，M组采用连续旋转运动镍钛器械M3-L进行一次性根管治疗，P组采用ProTaper机用镍钛锉进行一次性根管治疗。比较一次根管预备成功率、根管充填质量和适充率，治疗前、治疗1周后和治疗4个月后患牙疼痛评分变化，评价患者的治疗满意度。结果 W组、M组和P组一次根管预备成功率分别100.00%、98.04%、82.35%，W组和M组一次根管预备成功率均高于P组（P<0.05）；W组和M组适充率分别为100.00%和100.00%，均高于P组（80.39%，P<0.01）；治疗1周后和治疗4个月后W组、M组患牙疼痛评分均低于P组（P<0.05），且3组治疗1周后患牙疼痛评分均低于治疗前（P<0.05），4个月后患牙疼痛评分均低于1周后（P<0.05）；W组和M组对根管预备、根管填充、根管治疗后的满意度均高于P组（P<0.01）。结论 在后牙一次性根管治疗中，单支锉系统中Wave one Gold和M3-L比传统ProTaper机用镍钛锉的根管预备效果更佳，且能提高适充率，减轻疼痛感，提高患者满意度，Wave one Gold和M3-L之间的效果无明显差异。 |

| Objective The aim of this study was to explore the application value of single file system in single-visit root canal therapy of posterior teeth. Methods 153 patients were selected for single-visit root canal therapy of posterior teeth. They were randomly divided into group W, group M and group P with 51 cases in each group. Group W was given single-visit root canal therapy with Wave one Gold in single file system. Group M was given single-visit root canal therapy with reciprocating nickel-titanium instrument M3-L. Group P was given single-visit root canal therapy with ProTaper machine and nickel-titanium file. The success rate of root canal preparation, the quality and filling rate of root canal filling, and the changes inpain score of affected teeth before, after 1 week and 4 months were compared. The patients' satisfaction with treatment was evaluated. Results The success rates of single-visit root canal preparation in group W, M and P were 100.00%, 98.04% and 82.35%, respectively. The success rates of single-visit root canal preparation in group W and M were higher than that in group P (P<0.05). The filling rates of group W and M were 100.00% and 100.00%, respectively, higher than that of 80.39% of group P (P<0.01). The pain scores in group W and M after 1 week and 4 months were lower than those in group P (P<0.05). The pain scores in three groups after 1 week and 6 weeks were lower than those before treatment (P<0.05). The pain scores of affected teeth after 4 months were lower than those after 1 week (P<0.05). The pain scores of affected teeth in group W, group after 1 week and 4 months were lower than those in group P (P<0.05). The satisfaction rates of root canal preparation, root canal filling and root canal treatment in group W and group M were higher than those in group P respectively (P<0.01). Conclusion In single-visit root canal therapy of posterior teeth, the Wave one Gold and M3-L in single file system have a better effect on root canal preparation than the traditional ProTaper machine, which can improve the filling rate, reduce pain and achieve patient satisfaction, without a significant difference in effect betweenWave one Gold and M3-L. |
